Lethargic
Sleeps but easily aroused
Speaks and responds slowly and appropriately
Obtunded
Difficult to arouse
Returns to sleep quickly; may respond inappropriately
Stuporous
Aroused only through pain
No verbal response
Semicomatose
Responds only to pain
Gag and blink reflexes intact
Comatose
No response to pain
No reflexes or muscle tone Note: dying clients will proceed through these levels in this above-listed sequence
