Sequence for Open Gloving



1. Perform thorough handwashing.

2. Remove outer glove package wrapper by carefully separating and peeling apart sides.

3. Grasp inner package and lay it on clean flat surface just above waist level. Open package, keeping gloves on wrapper's inside surface.

4. If gloves are not pre-powdered, take packet of powder and apply lightly to hands over sink or wastebasket.

5. Identify right and left glove. Each glove has cuff approximately 5 cm wide. Glove dominant hand first.

6. With thumb and first two fingers of non-dominant hand, grasp edge of cuff of glove for dominant hand. Touch only glove's inner surface.

7. Carefully pull glove over dominant hand, leaving cuff and being sure cuff does not roll up wrist. Be sure thumb and fingers are in proper spaces.

8. Clench fist for sterelity.

NCLEX - Foley Catheter Procedure

1. Place the client supine with knees flexed
2. Open the sterile pack and maintain sterility
3. On the sterile pack, remove the water proof pad and placed it on the clients buttocks.
4. Put on the sterile gloves
5. Initiate the balloon of the catheter to check for leaks.
6. Lubricate the tip of the catheter.
7. cleanse the urinary meatus with anteseptic solution using a sterile forcep and cotton balls.
8. insert catheter. secure tubings on the bed and legs.
9. Wipe the urinary meatus with a cotton ball saturated with cleansing solution.

* The strategy to use to answer this kind of uestion is to picture yourself performing the procedure. First, prepare the client. Next, prepare the equiptment in the correct order using sterile technique. Open the sterile pack between the client's legs. Next, put on the sterile gloves. Inflate the balloon of the catheter to check for leaks. Lubricate the tip of the catheter.

Now the equiptment is ready.

Now, get the client ready for the insertion of the catheter. The last step from those provided is, wipe the urinary meatus with a cotton ball saturated with cleansing solution.

Sequence for Donning Personal Protective Equiptment (PPE)

1. Gown first
2. Mask or Respirator
3. Goggles or Face shield
4. Gloves

* The gown should be donned first. The mask or respirator should be put on next and properly adjusted to fit; remember to fit check the respirator. The goggles or face shield should be donned next and the gloves are donned last. Keep in mind, the combination of PPE used, and therefore the sequence for donning, will be determined by the precautions that need to be taken.



Sequence for Removing PPE

1. Gloves
2. Face shield or goggles
3. Gown
4. Mask or respirator

* The sequence for removing PPE is intended to limit opportunities for selfcontamination. The gloves are considered the most contaminated pieces of PPE and are therefore removed first. The face shield or goggles are next because they are more cumbersome and would interfere with removal of other PPE. The gown is third in the sequence, followed by the mask or respirator.



* The location for removing PPE will depend on the amount and type of PPE worn and the category of isolation a patient is on, if applicable. If only gloves are worn as PPE, it is safe to remove and discard them in the patient room. When a gown or full PPE is worn, PPE should be removed at the doorway or in an anteroom. Respirators should always be removed outside the patient room, after the door is closed. Hand hygiene should be performed after all PPE is removed.
